We are pleased to share Wellness Screenings will be available to eligible employees throughout Crown manufacturing, office, and branch locations. Screenings are voluntary, confidential and no cost to the employee. Participate in the annual HealthWise Wellness Screening Program to be eligible to receive a $130.00 incentive ($5.00/pay) for 2024. To qualify you must complete both a Wellness Screening and a Health Questionnaire by December 17, 2023.
On-Site Screenings: Click to view the screening schedule. All screenings, outside of manufacturing plants, require online registration.
Health Questionnaire: Log in to my.questforhealth.com to complete. Returning users will use the previous year’s log in credentials. New users will use Registration Key: CROWN23
