All full-time employees (with or without Crown healthcare coverage) are eligible to earn an additional $130 ($5/pay*) in 2021. Participation is voluntary.
To earn the HealthWise Credit (HWC), complete ONE of the following options:
Option 1 – Financial Wellness Tracker Assessment
Complete the online and confidential Financial Wellness Tracker Assessment offered through Merrill, Crown’s 401(k) Plan provider, before 3/31/2021.
- Log on to https://benefits.ml.com and click on the Financial Wellness tab located at the top.
Contact Merrill at 800-228-4015 if you have any trouble accessing your account.
- Complete the assessment and receive your wellness score.
- No further action is needed. Merrill will be providing HealthWise the names of individuals that have completed the assessment. All financial information is confidential and will not be shared with Crown.
- If you completed the assessment in 2020, you are eligible to earn the 2021 HWC.
Option 2 – Preventive Care Service
Submit documentation such as a receipt, Explanation of Benefits (EOB) or a HealthWise Acknowledgement Form completed by your Provider for a Preventive Care Service (flu shot, dental exam, etc.) completed between 1/1/2020 – 3/31/2021 to HealthWise.
