After reviewing the tab descriptions below, go to My.QuestforHealth.com then Register Now. Follow the prompts to create your account. (company code = CROWN) You will be asked to create a username and password. It is important you remember your username and password. You will be directed to your account when your online report is available.
WELCOME TAB – click on About My Program to view the Crown Screening Schedule. Click Get Started to go to next tab.
YOUR SCREENING TAB – click Participate Now. PLEASE READ EACH SCREENING OPTION CAREFULLY. New Bremen area employees that plan to attend a screening event in Plants 3, 4, 5, 6, 7, 10, 14, Celina, Minster, New Knoxville, or Ft. Loramie DO NOT schedule an online appointment.
HEALTH QUESTIONNAIRE TAB – click Complete Your Questionnaire. All questions must be answered. On the final question, be sure to click the Finish button to submit your questionnaire.
VIEW YOUR RESULTS TAB – this tab will display when your results are ready. Both the questionnaire and screening will need to be completed before you can view your results.
